Hyaluronic acid (HA), a linear polymer of glucuronic acid N acetyl glucosamine disaccharide, has tissue healing properties and is used for treatment of oral ulcers. This study aims to develop a controlled release mucoadhesive buccal film containing HA as a novel delivery system to overcome the drawbacks of the fast wash off of mouth rinses and gels used for treatment of aphthous ulcers. HA films are prepared by applying freeze and thaw cross linking technique. A full 21 × 31 × 41 factorial design was applied to study the effect of three independent variables namely polymer type, polymer concentration and the number of freeze/thaw cycles on the selected dependent variables of tensile strength, mucoadhesion and time required to release 50% of HA (T50%). The prepared HA films were evaluated for their physicochemical properties including folding endurance, surface pH, thickness, mucoadhesion properties, tensile strength and in vitro drug release. The optimum formula was evaluated for its clinical efficiency in treatment of patients have aphthous ulcer. The obtained results (good physical properties, mechanical properties (high TS), mucoadhesion properties and controlled release (high t50%)) indicated that an optimum formula for controlled release mucoadhesive buccal film which is aimed to be effective, safe and stable when used in treatment of aphthous ulcers. After clinical study, it was observed that the formulation of the hyaluronic acid resulted in a more potent effect and resistance to the washing effect of saliva and friction from food. The efficacy of hyaluronic acid was greatly affected by formulation which allowing for prolonged contact.

Effect of Mushroom Extract on 7,12-Dimethylbenz[a]anthracene Induced Salivary Gland Pathosis in Albino Rats

Salivary glands are essential structures for the wellbeing of an individual. Salivary gland tumors represent a diagnostic challenge. Following the treatment protocol for malignant salivary gland tumors, the majority of patients experience decreased salivation. Regeneration after the damage resulting from the treatment protocol, is believed to be through the transient activation of the Wnt/β-catenin pathway which preserve the stem/progenitor pool and allow for regeneration. Medicinal mushrooms have been tried in medicine for centuries. They have been reported to have anti-inflammatory, cardio-protective, hepatoprotective, and anticancer properties. Aim of the study: to evaluate the effect of mushrooms on the salivary glands of albino rats following DMBA induced pathological changes. Materials and methods: the study comprised three groups each of 12 albino rats. Group I was the control group, group II was given DMBA and group III was treated with mushrooms, following DMBA adminsteration. All groups were assessed histopathologically (H&E) and immunohistopathologically (PCNA). Results: group II exhibited variable histopathological signs from apoptosis to inflammation, allergy and dysplasia. Group III showed absence of some of the previous signs and a significantly higher PCNA expression. Conclusion: mushrooms may help in the regeneration of acinar cells through the activation of the progenitor cells. It also may have a cancer- protective role.

Role of Alpha Smooth Muscle Actin in Oral Squamous Cell Carcinoma Progression

: Squamous cell carcinoma (SCC) is the most common type of oral cancer. Malignant epithelal cells undergo cytological changes by a process referd to as epithelial mesenchymal transition (EMT).The cancer associated fibroblasts (CAFs) in the tumor micro-environment are now the focus of intense research and are believed to correlate with poor prognosis. They are characterized byalpha smooth muscle actin(α-SMA) expression, which is a myofibroblastic marker. Physiologically myofibroblasts are not as abundant as fibroblasts in the oral cavity, they have limited locations such as blood vessels walls in the oral mucosa. Aim: evaluation of the potential role of α-SMA in the progression of SCC. Materials and methods:qRT-PCR was performed to evaluate α-SMA gene expression in tongue SCC cell line (SCC 25), normal fibroblasts cell line (Wi-38) and in a coculture of both cells.Qualitative and quantitative immunohistochemical analysis of α-SMA expression withclinico-pathological correlations was performed for24 SCC specimens of different grades. Results:qRT-PCR results showed that there was α-SMA expression in the epithelial cells, and that co-culturing resulted in an overall increase in α-SMA expression. Immunohistochemical evaluation of the specimens revealed that the expression of α-SMA increased with tumor grade and correlated with lymph node involvement.Conclusion:α-SMA can be used as a prognostic marker and a potential target for cancer therapy.

Potential Therapeutic Effect of Moringa Oleifera on Tongue papillae of Diabetic Albino Rats

Background: The tongue is the second most commonly affected oral site after periodontal tissues in diabetes. Diabetic complications are mainly attributed to oxidative stress and side effects from long term use of drugs to treat diabetes. MO has been reported to be a valuable source of natural antioxidants. Materials and methods: this study comprised three groups; control, diabetic and MO treated groups. The experiment was terminated after fourteen days. Evaluation of the rats was done by measuring blood glucose levels, as well as, histological assessment of the tongue tissue by H&E and SEM. Results: There was a significant reduction in blood glucose levels and histological evaluation revealed improvement in the condition of the tongue mucosa in the MO treated group compared to the diabetic group. Conclusion: MO has a hypoglycemic effect on the blood glucose level of diabetic albino rats. MO may partially reverse some of the detrimental effects hyperglycemia has on the tongue mucosa of diabetic rats.

CD44 is a cell adhesion molecule that functions in cell proliferation, differentiation, migration and angiogenesis, as well as in signaling for cell survival. CD44 acts also as the main hyaluronan receptor. CD44 has been implicated to have a role in carcinogenesis. Aim of the study In this study the expression of CD44 in mucoepidermoid carcinoma was detected and correlated with the histopathological grade. Material and methods peroxidase-antiperoxidase immunohistochemical technique for the detection of CD44 expression in mucoepidermoid carcinoma. Results There was no statistically significant difference between the expression of CD44 in high and low grade mucoepidermoid carcinoma. Yet, the pattern of expression varied in both. Conclusion CD44 pattern of expression correlates with mucoepidermoid carcinoma histopathological grade. Correlates with increased proliferative activity and presumably increased cellular motility. Summary From this study it was deducted that over-expression of CD44 correlates with increased proliferative activity and presumably increased cellular motility.

Synergistic Effect of Combined Treatment with Extra Virgin Olive Oil and Doxorubicin on squamous cell carcinoma cell line

Background: In spite of the fact that the medical care has been extremely improved, the survival rate of patients diagnosed with Squamous cell carcinoma is still decreased . Doxorubicin (DXR) is one of the most effective anti- squamous cell carcinoma drugs. This drug has a dangerous side effects. Natural products can lighten the DXR- induced side effects, without compromising its antitumor efficacy. The consumption of extra virgin olive oil (EVOO), appeared to be related to fewer malignant tumor occurrences. Aim of the study: The objective of the present study was to evaluate the effect of EVOO when used alone and, more interestingly, when combined with DXR, for 48 hours, on lingual squamous cell carcinoma cell line (SCC25). Materials and methods: SCC25 was used in this study, it was divided into four groups. Group I (control), group II (DXR treated), group III (EVOO treated) and group IV (EVOO+DXR treated). Results: there was significant decrease in MMP-1, VEGFR-2 expressions as well as migration and invasion between the test groups and the control group. Conclusion: EVOO is a promising anticancer compound when used alone or in combination with DXR.

Effects of Moringa Oleifera Aqueous Leaf Extract on Submandibular Salivary Glands of Diabetic Albino Rats

Moringa Oleifera (MO), also known as the ‘drumstick tree,’ is recognized as a nutritious and cheap source of phytochemicals, that a have a prominent anti-oxidative effect. Salivary gland dysfunction has been frequently reported in diabetic patients. Diabetes is a chronic metabolic disease that has complications mainly resulting from persistent hyperglycemia. Aim of the study: To Assess the effect of MO aqueous leaf extract on blood glucose levels in diabetic albino rats and its effect on submandibular salivary glands of diabetic albino rats. Materials and methods: the study comprised three groups; control, diabetic and MO treated groups. The experiment was terminated after fourteen days. The evaluation was performed by measuring the blood glucose levels and weight. Histological evaluation was done by H&E, PAS and IHC for COX-2. Results: the blood glucose levels and histological signs of diabetic complications were significantly lowered in the MO treated group. Conclusion: MO is a promising anti-diabetic treatment and may even reverse some of the diabetic complications.

Possible synergistic anti-carcinogenic effect of Moringa Oleifera and Cisplatin on human squamous cell carcinoma

Introduction Moringa Oleifera (MO) is a cheap source of glucosinolates, falvonoids, among other components that may be used in the treatment of various illnesses including cancer. Cisplatin is a widely used anticancer chemotherapeutic agent. More than 90% of all oral cancers are squamous cell carcinoma (SCC). In spite of the advances in treatment modalities the survival rates have not improved. Aim of the work: The aim of the present work was to assess the anticarcinogenic potentiality of MO extract alone and when used in conjunction with different doses of Cisplatin in treatment of human squamous cell carcinoma cell line. Materials and methods: MO leaf extract, Cisplatin® 10mg / 10 ml 1 vial and HEp2 cells were used in the present study. In the present study, Cisplatin 20 mg, 50 mg and MO extract were used independently and in combination on HEp2 cell line. The expressions of caspase 9 and Beta catenin, and viability were assessed after 24 and 48 hours. Results: Phase contrast microscopic results showed variations in morphology and number of cells between different subgroups. Highest Caspase 9 gene, least Beta catenin and least viability were noted when HEp2 cells were treated with Cisplatin 50 mg and MO for 48 hours. Conclusions: MO has an effective anticarcinogenic role on HEp2 cell line. The combined use of MO and Cisplatin is more effective than either of them alone, in a dose and duration dependant manner.

EFFECT OF HUMAN BONE MARROW DERIVED MESENCHYMAL STEM CELLS ON SQUAMOUS CELL CARCINOMA CELL LINE

Back ground: Researchers have been evaluating the use of stem cells in anticancer therapy for several years. Squamous Cell Carcinoma (SCC) is considered the most common epithelial malignant neoplasm.There is an over expression of Bcl2 and Survivin and VEGF in SCC which correlate with more aggressive tumors that do not respond to treatment. Material and Methods: This study consisted of five groups. The first group consisted of HEp2 culture that acts as a control. The second and third groups consisted of HEp2 cells cultured in MSCs conditioned medium for 48 and 96 hours, respectively. The fourth and fifth groups consisted of HEp2 cells cultured in co-culture conditioned medium for 48 and 96 hours, respectively. All cultures were evaluated for Survivin, Bcl2 and VEGF gene expression using real time PCR and proliferation rates using MTT assay. Results: The results showed that culturing HEp2 in MSCs conditioned media and in co-culture conditioned media for 48 hours reduced Survivin, Bcl2 and VEGF gene expressions when compared to their expression in the HEp2culture. There was a further reduction in Survivin and VEGF gene expressions when culturing in these media was extended until 96 hours while Bcl2 expression increased. Conclusion: MSCs and Cocultured media reduced proliferation rate and angiogenesis in HEp2 cell line, while its effect on apoptosis still needs further studying.
